Care Partners of America, Inc. was founded in April 2001 because of a need that was not currently being met for an extremely large patient population. The need was Home-Based Physical Rehabilitation Services. Today, Care Partners of America is extending the healing mission into the home by offering in-home rehab services in various cities in Mississippi, Alabama and Tennessee. By providing a smooth transition for continuity of care to patients who are discharged from in-patient and out-patient treatment facilities, Care Partners is reducing or eliminating the need for nursing home placement or rehospitilization for many patients being treated.

Typically, the service provided by Care Partners has only been available through home-health agencies, where a patient must meet strict federal guidelines that require home-bound status along with other medical criteria. By bridging the gap between outpatient rehab treatment facilities and home health services, Care Partners has been and will continue to be committed to restoring health and restoring life. Because life is not being alive, but living well.
