was founded in April 2001 because of
a need that was not currently being
met for an extremely large patient population.
The need was Home-Based Physical Rehabilitation
Services.
Today, Care Partners of America is extending
the healing mission into the home by
offering in-home rehab services in various
cities in Mississippi, Alabama and Tennessee.
By providing a smooth transition for
continuity of care to patients who are
discharged from in-patient and out-patient
